Also known as the amended individual tax return form, it is used to correct any errors in income, reimbursements or tax credits on a number of other Connecticut CT-1040 tax documents. When filling out your CT-1040X form, you will be given the chance to explain why you are making an amendment in your tax return.
Connecticut Trusts and Estate Income Tax Return
Form CT-1041 may be filed only by resident estates, trusts and part-year resident trusts that need to fill their income tax return. If you are a resident estate, trust or part-year resident trust and you need to include your payment, you need to write “2013 Form CT-1041” and the Federal Employer Identification Number (FEIN), if applicable, or SSN (optional), on the front of your check.
Connecticut Underpayment of Estimated Income Tax by Individuals, Trusts and Estates (Schedules A & B)
Form CT-2210 can be filed by individuals, estates and trusts. Connecticut Form CT-2210 is used by those who have submitted Forms CT-1040, CT-1041 and CT-1040NR/PY who paid less than their estimated income tax. This form is used to calculate the amount of interest due, or to lower or eliminate returns that would otherwise apply.
Connecticut Individuals Alternative Minimum Tax Return
Form CT-6251 may be filed by all individuals who are residents or part-year residents and have a federal alternative minimum tax liability or by individuals who are non-residents with a federal alternative minimum tax liability and an income sourced from Connecticut. Connecticut Form CT-6251 must be filed even if your alternative minimum tax was zero.
Connecticut Credit for Prior Year Connecticut Minimum Tax for Individuals, Trusts, and Estates
Connecticut Form CT-8801 may be filled by trusts, individual and estates to compute adjusted net minimum tax credit for the Connecticut alternative minimum tax paid in prior taxable years. In order to determine the sum of credit carryforward you need to attach Form CT-8801 to the back of the Form CT-1040, Form CT-1041 or Form CT-1040NR/PY.
Connecticut Earned Income Tax Credit Self-Employed Questionnaire
Form CT-EITC may be filed by taxpayers only if the Department of Revenue Services (DRS) requests it. You should mail the Connecticut Form CT-EITC and all related documents to the Department of Revenue Services State of Connecticut, PO Box 2980, Hartford, CT 06106-2980.
Connecticut Income Tax Withholding Supplemental Schedule
Schedule CT-1040WH must be completed only if you have more than seven 1099, W-2 or Schedule CT K-1 forms. Once completed, you will need to attach Schedule CT-1040WH on the back of Form CT-1040NR/PY, Form CT-1040 or Form CT-1040X.
Connecticut Earned Income Tax Credit
Schedule CT-EITC must be completed only by Connecticut full year residents who earned income from Connecticut sources but did not earn a high income during the current taxable year. Qualified individuals may pay less federal tax or no tax at all.
Connecticut Nonresident or Part-Year Resident Schedule of Income From Connecticut Sources
Schedule CT-SI may be used only by non-residents or part-year residents to file their taxable income from Connecticut sources. Connecticut Schedule CT-SI must be completed and attached to the Form CT-1040NR/PY and mailed to the Department of Revenue Services State of Connecticut.
